Abstract
Readmission risk screens are national standards of practice for case managers and will soon be required by pending federal regulations. To improve quality metrics as well as meet federal regulations and professional standards, Children’s Mercy Hospital is developing the HARRPS (High Acuity Readmission Risk Screen) Tool, a research-based index admission tool to screen for risks associated with pediatric readmissions. This article will discuss both the tool and the quality improvement processes used to increase employment of the tool and to standardize its use.
